Duke marrë parasysh se sistemi shëndetësor është një ndër sistemet më të rëndësishme të shoqërisë në një vend, qëllimi kryesor i të cilit është të përmirësojë dhe ruajë shëndetin e njeriut. Gjithashtu si institucion publik shëndetësor përbëjmë mbrojtjen shëndetësore primare të popullatës duke marrë parasysh se një sistem efikas i kujdesit shëndetësor është thelbësor për zhvillimin e suksesshëm të shoqërisë. Zhvillimi teknologjik dhe socio-ekonomik inkurajon forcimin e sistemit shëndetësor, i cili nga ana e tij çon në përmirësimin e shëndetit të popullatës. Qëllimi kryesor i ketij punimi është që të bëj analizën e pasqyrave financiare dhe strukturore në bazë të njësive të Institucionit publik shëndetësor Shtëpia e Shëndetit-Gostivar për vitet 2017-2021. Ky institucion është i finacuar nga mjetet buxhetore dhe ato vetanake, si financues i këtyre shëbimeve është Fondi Shëndetësor i RMV-së, gjithashtu matjen e performancës dhe efikasitetit të funksionimit si institucion shëndetësor që mbulon shërbime primare dhe sekondare shëndetësore për qytetarët. Qëllimi bazë i këtij punimi është analizimi i pasqyrave financiare si dhe analiza e performancës financiare të njësive organizative dhe të institucionit publik shëndetësor Shtëpia e Shëndetit-Gostivar prej vitit 2017-2021 . Analiza synon të jep përgjigje bazike në hipotezëen kryesore dhe hipotezat ndihmëse në kuptim të konstatimit të shkaqeve për perfomancën financiare asimetrike të njësive organizative (disa punojnë me fitim dhe të tjera me humbje) përkundër faktit se institucioni publik shëndetësor Shtëpia e Shëndetit-Gostivar gjatë periudhës së analizuar 2017-2021 tregon rezultat financiar pozitiv. Punimi synon të rekomandojë reforma financiare dhe organizative në kuptim të tejkalimit të pozicioneve problematike në Bilancin e Gjendjes dhe rritjen e financimit nga Fondi Shëndetëor për shërbimet e domosdoshme shëndetësore ose instalimin e participimit financiar nga qytetarët dhe subjektet që deri tash kanë shfrytëzuar falas këto shërbime shëndetësore që janë përgjegjësi e njësive organizative dhe shtëpisë së shëndetit. Autori rekomandon që krahas stabilizimin financiar duhet të rriten investimet kapitale të cilat kanë nivele minimale në rritjen e investime me teknologji të re moderne në objekte dhe në veçanti në pajimet shëndetësore në funksion të realizimit të shërbimeve shëndetësore më cilësore dhe në kohë ndryshime në rritjen e cilësise së resurseve humane (personel mjekësor të ri dhe të kualifikuar ). Autori mban bindjen se ky studim i parë në këtë institucion nxjerr në sipërfaqe dobësitë organizative dhe financiare dhe rekomandon alternativë për avansim të performancës së institucionit në fjalë.
АПСТРАКТ
Имајќи предвид дека здравствениот систем е еден од најважните системи на општеството во една земја чија основна цел е подобрување и одржување на здравјето на лугето. Исто така, како јавна здравствена установа ја сочинуваме примарна здравствена заштита на населението, имајќи предвид дека ефикасен здравствен систем е сусштински за успешен развој на општеството. Технолошкиот и социо-еконномскиот развој го поттикнува зајакнувањето на здравствениот систем, што пак води кон подобрување на здравјето на населението. Основата цел на овој труд е да ги анализира финансиските и струкурните извештаи на организационите единици на јавната здравствена установа Здравствен Дом-Гостивар за годините 2017-2021 година. Оваа институција се финансира од сопвствени ресурси како и од буџетски средства по основ на реализација на здравствени услуги,бидејќи финансиер на овие услуги е Фондот за Здравство на РСМ, како и мерењето на успешноста и ефикасноста на работењето како здравствена установа која ги покрива примарните и секундарните услуги здравје за граѓаните. Трудот има цел и ќе препорача во Билансот на државата и зголемување на средствата од Фондот за здравство за потребите на здравствени услуги или инсталирање финансиско учество од страна на граѓаните и субјектите кои користеле овие услуги досега бесплатно здравје кои се во надлежност на организациските единици и здравствените домови. Авторот препорачува, покрај финансиската стабилизација, да се зголемат и капиталните инвестиции кои имаат минимални нивоа во зголемувањето на инвестициите со нова современа технологија, капацитети и особено во здравствена опрема во функција на реализација на најважните здравствени услуги и во временски промени во зголемувањето на квалитетот на човечките ресурси (млад квалификуван медицински персонал). Авторот смета дека оваа студија, прва од овој вид за ова институција ги открива финансиските и организациските слабости и препорачува алтернативи за унапредување на работата на предметната институција во преспектива.
ABSTRACT
Considering that the health system is one of the most important systems of society in a country whose main goal is to improve and maintain human health. Also, as a public health institution, we constitute the primary health protection of the population, taking into account that an efficient health care system is essential for the successful development of society. Technological and socio-economic development encourages the strengthening of the health system, which in turn leads to the improvement of the health of the population. The main purpose of this paper is to analyze the financial and structural statements based on the units of the public health institution Gostivar Health House for the years 2017-2021. This institution is financed from its own resources as well as from budget resources based on the realization of health services, as the financier of these services is the Health Fund of the Ministry of Health, also the measurement of the performance and efficiency of the operation as a health institution that covers primary and secondary services health for citizens. The basic purpose of this paper is to analyze the financial statements as well as the analysis of the financial erformance of the organizational units and the public health institution Gostivar Health Center for the period 2017-2021. The analysis aims to provide basic answers to the main hypothesis and adinational hypotheses in terms of ascertaining the causes for the asymmetric financial performance of organizational units (some work with profit and others with loss) despite the fact of the public health institution Gostivar Health Center from the analyzed period 2017-2021 shows a positive financial result. The paper aims and will recommend organizational and financial reforms in the sense of overcoming the problematic positions in the Balance of the State and increasing the funding from the Health Fund for the necessary health services or the installation of financial participation by citizens and entities that have used these services for free until now. The author recommends that in addition to financial stabilization, capital investments should be increased, which have minimal levels in the increase of investments with new modern technology, facilities and especially in health equipment in the function of the realization of the most important health services and in time changes in the increase of the quality of human resources (young and qualified medical personnel).
